This exclusive Class Two series covers successful dental materials and techniques. Double-cord impression technique is the gold standard.
Ferric Sulfate Hemostatic Scrub
The strongest hemostatic agents have a ferric ion active ingredient. BEWARE of the stain that it will cause so it should not be used when bonding.
“Hemostatic Agents on the DL”Choose Your Cord
- Woven cord necessitates expansion before it will absorb hemostatic agent. Alumina-ion based hemostatic agent is the right choice in this step.
- Assuming good periodontal health, standard bottom cord = “000” or “00” & top cord = “0” or “1”
“Roll With It”
- Fiber slides most easily against hard surfaces. The cord packing instrument should start at a perpendicular angle to long axis of tooth and then rotate to parallel.
- Start with the deepest pocket and “pack back”
Why We Use 2 Cords
- Bottom cord integrates with the clot of sulcus. This cord should be the exact circumference of the tooth.
- Top cord expands tissue, differentiating the prepared margin — since the second cord will be removed, it should have a tail
PVS and Moisture
PVS is impossible to remove from a dry surface. Make your tooth dry for that reason. High air blast for 5 sec before you start dispensing impression material around the tooth. HIGHLY RECOMMEND only 1 time around the sulcus to avoid bubbles. Do not blow air.
“PVS: The Make or Break 42 Seconds”Caterpillar Tray Fill
The bane of existence is a non-critical area becoming critical. Caterpillar technique (vs. sweeping technique) eliminates more bubbles.
Static Tray Position
Hold on to the tray in the patient’s mouth until the PVS is set. Pressure and lack of support cannot be detected in the impression but the distortion will cost you precious time on delivery day.
Evaluate & Repeat If Not 100%
The sulcus has been supported by impression material, so if you need to take another, do it within 1–3 min without packing new cord.
“Your Double-Cord Impression was a NO-GO…Now What?”Anatomy of a Good Impression
- Margin needs to be 100%
- Axial walls should be able to be approximated
The Impregnated Cord
Many times the cord comes out with an excellent impression. DO NOT PULL! Cut the cord short of critical areas to avoid ripping impression material.
